Person Centred Planning facilitates the understanding of the lifestyle desired by the person and how it may be achieved. It is value-laden and rooted in a profound respect for the individual and an expectation that the individual will be included in her or his community. This requires sufficient time to discover the core values of the individual and to ensure that these values are accounted for in the plan.
The vision that arises from a care plan is the individual's vision, not the vision of the professionals or the system. Describing someone else's vision of their future requires that we understand the individual and their core values. Understanding an individual is never complete; people continuously change. The understanding that we do achieve arises from what is shared.
Person-centred plans concentrate on the need for us to listen to people and their potential and make sure we also monitor their advancement in terms of quality of life.
We endeavour to focus on what steps forward means for the individual and will produce an achievable person centred plan that states exactly how we will support each service user, the standards and outcomes to be achieved and who will deliver each aspect of the plan. This will also identify the resources we need to provide for each person's needs.
We also ensure that we will work with the person's 'circle of support' which includes greater involvement with their families. All our service users have a copy of their PCP's and we ensure that any information we give is in a meaningful way to that person and is in an easy to read format.
The support plan focuses on what is important in everyday lives: people's routines, rituals, who they spend time with, their pace of life. It also identifies supports and how those supports should be provided to help the person live more of the kind of life that they want.
