Our Formulation Approach:
The approach at John Munroe Group (dependent on presentation and compliance at the time) would be to set out a behavioural management plan, which will be led by a Consultant Psychiatrist and a Consultant Psychologist.
The initial approach will be to map the behaviours for the first 6 – 8 weeks until the first Care plan review meeting between week 6 to 8.
There will be regular MDT reviews including psychological, OT, Nursing and Medical assessments during that period and ongoing until discharge.
If the patient is assessed as suitable for psychological intervention, a comprehensive psychological assessment will take place followed by a psychological formulation meeting where the plans for management will be explored and initiated.
Discharge planning and treatment goals will be formulated at the first CPA around week 6, the ultimate aim would be to determine what level of community care is required (usually 24 hour nursing care or supported living) and monthly goals to be reviewed at MDT with a plan to minimise challenging behaviours, optimise functioning and facilitate discharge with an appropriate plan for the nursing home or supporting living to implement.
